By registering for this program I am accepting the waiver of rights and that there are inherent risks associated with physical activity and participating in dance classes. I understand, take responsibility for all risks and give permission for myself and my child to participate in Markham School of Dance’s programs without restrictions. I hereby release Markham School of Dance, MainStreet Dance, MainStreet Dance Company, and any of the teachers, assistants, volunteers, students and associates of any and all liabilities. I understand that Markham School of Dance is not responsible for the use of the indoor playground and their facilities. An additional waiver is required to be signed upon entering the facillity for this event. 

Photo and Video Release: I authorize Markham School of Dance to copyright or publish any photographs or video footage taken of myself or my child while participating at this event and Markham School of Dance may use these without limitations or reservations.
