By registering for this program I am accepting the waiver of rights and that there are inherent risks associated with physical activity and participating in dance classes. I understand, take responsibility for all risks and give permission for myself and my child to participate in Markham School of Dance’s programs without restrictions. I hereby release Markham School of Dance, MainStreet Dance, MainStreet Dance Company, and any of the teachers, assistants, volunteers, students and associates of any and all liabilities.

Photo and Video Release: I authorize Markham School of Dance to copyright or publish any photographs or video footage taken of myself or my child while participating as a student of Markham School of Dance, and Markham School of Dance may use these without limitations or reservations. I have read and agree to all the terms and conditions as set by Markham School of Dance.

I agree to the following:

EPIPENS and Asthma Inhalers: if your child requires these you must bring them and check them in with the teacher each day. If they are expired or not available, you will not be able to bring your child to camp that day.

CAMP FEES: Fees are non-refundable within 8 days of the camp start date. Prior to that it is refundable less a $25 administration fee. If there is availability in another camp week you may be able to switch weeks if needed.

SICKNESS: I understand that if students are not feeling well they are not able to attend the camp program. There are no refunds due to student sickness however if there is another camp in the same year with spaces available you may be able to attend. Arrangements must be made through the office.

Markham School of Dance reserves the right to modify/merge reschedule classes due to low enrollment.
