By registering for this program I am accepting the waiver of rights and that there are inherent risks associated with physical activity and participating in dance classes. I understand, take responsibility for all risks and give permission for myself and my child to participate in Markham School of Dance’s programs without restrictions. I hereby release Markham School of Dance, MainStreet Dance, MainStreet Dance Company, and any of the teachers, assistants, volunteers, students and associates of any and all liabilities.
Photo and Video Release: I authorize Markham School of Dance to copyright or publish any photographs or video footage taken of myself or my child while participating as a student of Markham School of Dance, and Markham School of Dance may use these without limitations or reservations. I have read and agree to all the terms and conditions as set by Markham School of Dance.
I agree to the AUTO BILLING as follows:
Class Fees - Your first payment is charged upon registration and includes the first monthly payment, the class uniform, shoes and a registration fee. Monthly billing is charged on the date of your registration. If for any reason you need to withdrawal you can stop the next billing payment by selecting "Manage Payment" from your invoice. There are NO REFUNDS for payments made or missed classes and there are NO MAKEUP CLASSES if you miss a class.
Performance fees - There are three recitals per year - December, and May/June and August. Performance fees including costumes, photos, videos and are not included in the class fees. Additional fees for these events start at $100 per dance.
Competition fees - these start at $50 per group dance up to $125 per solo. The exact fees will be announced later in the season. These will be autobilled at a later date.
Other additional fees may include: workshop fees, dance team jackets and additional dancewear.
STUDIO UNIFORM:
I understand that the studio has a uniform and all students will be provided with the dress code items when they register. Additional items may be purchased by the studio. The studio uniform is required of all students.
